The hip joint is the most proximal joint in the lower limb. It is a relatively stable joint that is more prone to degenerative changes and bone disease. Disorders of the hip are more commonly age related than any other joint however it is also seen in young children in the form of inflammatory disorders. Referrals are common in the hip area and a careful asessment of the patient is needed when forming a working diagnosis.

As a structural chiropractor normal spine alignment is key to making sure that forces on muscles ligamenst and connective tissues are symmetrically distributed evenly through both legs. What we commonly find with hip pain is that it can be caused by an acute or chronic onset and the majority of time it is associated with abnormal loading of the bones, joints and supportive tissues. Quite often medicine will start with anti inflammatories and pain killers. This only treats the symptom and may fail to address the cause.

The first approach that a chiropractor should do is a thorough postural assessment. This should be done from the front, back and side views. Imblances in posture will be evident even to the patient when viewing their posture in a mirror. If postural deviations are found the most accurate way to determine spinal alignment is with a standing x-ray. Something to note is that lay down x-rays change the alignment of the spinal in coth a saggital and coronal plane and are innacurate for looking at alignment issues of the spine. A secondary test may be with a force plate device that measures load on the florr. Imblanaces in posture with show imbalances in pressure plate could show a weight difference on one side. This is not always the case though.

Orthopaedic tests can confirm sites of irritation in tissues but will not should alignment issues. These tests are useful in drilling down to the specific tissue in lesion or symptomatic area.

A thorough x-ray analysis needs to be done on the x-rays and the best software on the market at this point is Posture Ray. A written report will be given to the patient explaining the differences of their posture to the ideal. Any deviation away from the ideal position will inadvertently cause abnormal loads to be distributed through the area.

The primary focus of a corrective chiropractor will be to improve the alignment of the spine and normalise the load on the surrounding tissues. The patient will also need to be educated in home exercises and stretches to assist with the corrective process. Working ergonomics as well as home ergonomics will also need to be addressed.

The patient is then given two choices for their health goals. Pain Relief or Corrective Care. Pain Relief focusses on reducing tyhe patients symptoms which may range from 1-12 adjustments. A Pain Program does not focus on correcting misalignments. Corrective Care on the other hand is designed to gently re-align the spine with specific corrective adjustments in conjunction with postural stretching and traction.

A treatment schedule will be set up based on scientific clinical guidelines and a re-assessment at intervals to assess the patients responsiveness to care. Pain questionaires can also be given to objectively measure improvement. In order to truly determine the effectiveness of care a post x-ray should be taken to determine if the patient has reached their health goal.

Chiropractic is a wholistic approach that focsses on reducing physical load on the spine and nervous system, reducing sress and making sure the patient knows what foods to eat in order to give the body what it needs to heal.
